As leaders it is always our responsibility to train our subordinates. We continually strive to teach our skills and knowledge to our subordinates. however; one key area we tend. to forget is recurrence prevention. It is our responsibility to document our experiences and histories so we can teach in the future to others so that they will not repeat the same mistakes.

1. Process is performing well.
2. A problem or abnormal condition in the process occurs….performance goes down / dips to sub optimal.
a. Leadership steps in to solve the cause of the problem and restores performance, as well as learn and document.
3. Process performance goes back up to standard.
